The Insert key is tracked by each application itself. So you can have it enabled in one app, and disabled in another app. The 4dots app can't tell the Insert status for each app, so it just assumes it's off when 4dots starts, and then toggles the status each time you press the key.

Where that becomes a problem is a scenario like this:

• You have an app already open, with Insert turned on
• You open 4dots, it shows Insert is disabled
• You press the Insert key, now 4dots shows it's enabled, but the app has it disabled
